알파태아단백 (Alpha-fetoprotein; AFP)

알파태아단백은 태아의 간과 조류 알의 난황낭과 유사한 발생하는 배아의 일부에서 주로 생산되는 단백질이다. 알파태아단백의 농도는 생후 감소하기 시작하여 18개월이 지나면 성인에서 관찰되는 수치까지 감소한다. 건강한 어린이와 임신하지 않은 성인에서 알파태아단백은 매우 낮은 수준에서 검출 가능할 뿐이다.

간암

간암에 걸린 환자의 60-70%에서 정상보다 상승하기 때문에 간암을 선별하고 진단하는데 유용한 검사이다. 수치가 500 이상 (정상치는 10 이하)으로 나타나고 초음파 검사나 CT 촬영으로 종괴가 관찰되는 경우에는 조직검사 없이도 간암 진단을 내릴 수 있다.

신경관 결손증

신경관 결손증 (Neural Tube Defect)이 있는 태아는 두개골과 뇌막이 없으므로 남아 있는 뇌 부분이 양수에 노출되어 산모 혈액을 이용하는 기형아검사에서 태아 알파 단백 수치가 매우 높게 올라간다. 태아의 신체 내부가 양수에 노출됨으로 태아 알파 단백질이 양수를 통하여 산모 혈액으로 많이 들어가기 때문이다.

엄마의 혈액에서 알파태아단백으로 검사했을 때 개방성 신경관 결손증 (Neural Tube Defect)을 발견하는 율은 80-90%이다.

당뇨병을 가진 엄마의 태아에서 신경관 결손증의 발생률이 증가한다.

  • 태아의 임신 주수 : 태아의 임신 주수에 따라 정상 범위가 달라지므로 태아의 임신 주수가 잘못 계산된 경우 결과가 비정상으로 나올 수 있다.
  • 태아 수 : 알파 태아단백이 비정상적으로 높은 경우 혹시 쌍둥이가 아닌지 확인해 보아야 한다. 쌍둥이에서는 알파 태아단백을 두 태아가 만들어 내므로 수치가 높게 나온다.
  • 산모의 비만, 당뇨, 인종 등이 결과에 영향을 줄 수 있다.

1-2주 후 다시 재검사하고 계속 상승되어 있으면 초음파검사를 실시한다.

A multiple of the median (MoM) is a measure of how far an individual test result deviates from the median. MoM is commonly used to report the results of medical screening tests, particularly where the results of the individual tests are highly variable.

MoM was originally used as a method to normalize data from participating laboratories of Alpha-fetoprotein (AFP) so that individual test results could be compared. 35 years later, it is the established standard for reporting maternal serum screening results.

As an example, Alpha-fetoprotein (AFP) testing is used to screen for a neural tube defect (NTD) during the second trimester of pregnancy. If the median AFP result at 16 weeks of gestation is 30 ng/mL and a pregnant woman's AFP result at that same gestational age is 60 ng/mL, then her MoM is equal to 60/30 = 2.0. In other words, her AFP result is 2 times higher than "normal."

Results to the right of the blue line (a cutoff of 2.5 MoM) would be interpreted as "abnormal" while an AFP MoM to the left of the line would be considered "normal." Note that there is no single MoM cutoff that can completely separate unaffected from affected fetuses.
