======POP-Q System(Pelvic organ prolapse quantification system)======

[[med:pelvic_organ_prolapse|]]에서 객관적인 질병의 중증도를 평가하기 위해 만들어진 측정 체계.

* 1996 ICS 에서 POP-Q 발표
  * 실제 사용은???
     * 너무 시간이 오래 걸린다!
     * 너무 복잡하다!
  * 이해를 하면…	
     * 쉽고, 빠르고, 해설적이고, 재현 가능한 측정방법이다.
     * 의료진 간의 정확한 정보전달, 수술 효과의 분석, 연구간의 상호 비교에 필수적이다.

=====측정법=====

* 방광을 비운 상태에서 측정합니다. 
  * 환자의 자세 (Lithotomy or standing or left lateral)을 기록합니다. 
  * 측정은 항상 cm 단위로 기록하며, 0.5cm을 최소단위로 합니다. 
  * 총 9개의 수치를 측정해야 합니다. 
  * 이 중 5개 GH, PB, C, D, TVL 는 비교적 이해하기가 쉽습니다. 
  * TVL을 빼고는 항상 최대한 힘을 준 상태에서 측정합니다. 
  * 측정의 기준점은 처녀막 (혹은 그 흔적) (만약 평평하지 않다면, 3시나 9시방향이 기준)

===GH (Genital Hiatus)===
외요도구( external urethral meatus) ~ 처녀막의 뒷쪽 부분 (the posterior aspect of the hymenal remnant)까지 (최대한 힘을 준 상태에서)

===PB (Perineal body)===
처녀막의 뒷쪽 부분 (the posterior aspect of the hymenal remnant) ~ 항문의 중심(the middle of the anal opening) 까지 (최대한 힘을 준 상태에서)

===Point C (lowest part of cervix)===
처녀막을 기준으로 한 자궁경부의 위치 (가장 내려온 부분- os가 될수도 있고 ant. lip혹은 post. lip이 될수도 있다.)
자궁절제를 한 경우 질을 봉합한 부분이 Point C (D가 아님!)
처녀막보다 높으면 음수, 더 낮으면 양수.

===Point D (insertion of uterosacral ligament)===
처녀막을 기준으로 한 천골자궁인대(uterosacral ligament) 가 붙는 부위의 위치
대개 딱딱하게 만져지는데, 조직 손실이 심하여 만져지지 않을 경우 질 후방원개 (Post. fornix)로 대체.

===TVL (Total vaginal length)===
탈출증을 회복시켰을 때 처녀막을 기준으로 천골자궁인대(uterosacral ligament)가 붙는 부위(Point D)의 위치.

힘을 뺀 상태에서 측정한다. (유일!)
탈출한 것을 회복시킨 상태에서 두 손가락을  질 후방원개 (Post. fornix)에 넣고 측정.

자궁절제를 한 경우 질을 봉합한 부분 (Point C)을 측정

===Point Aa===
처녀막을 기준으로 질 앞벽의 -3cm(상방)지점이 힘을 줬을 때 하강하여 어느 위치에 있는지 측정.
-3cm 이면 그대로 있는 것이므로 정상. 0 이면 처녀막 까지 내려온 것.. (-3cm ~ 0 ~ 3cm)

===Point Ba===
처녀막을 기준으로 Point Aa 윗쪽의 질벽 중 아무 곳이나 제일 아래로 내려온 부분의 위치를 측정. (-3cm ~ 0 ~ )

===Point Ap===
처녀막을 기준으로 질 뒷벽의 -3cm(상방)지점이 힘을 줬을 때 하강하여 어느 위치에 있는지 측정.
-3cm 이면 그대로 있는 것이므로 정상. 0 이면 처녀막 까지 내려온 것.. (-3cm ~ 0 ~ 3cm)
===Point Bp===
처녀막을 기준으로 Point Ap 윗쪽의 질벽 중 아무 곳이나 제일 아래로 내려온 부분의 위치를 측정. (-3cm ~ 0 ~ )
