1. 췌내분비종양이란

췌장의 기능은 소화액인 췌액을 분비하는 외분비기능과 각종 호르몬을 생산하는 내분비기능으로 크게 나눌 수 있습니다. 외분비기능을 담당하는 영역은 췌장 전체의 95%를 차지하며, 내분비기능을 담당하는 것은 췌장 내에 산재하는 랑게르한스섬으로서 전체의 5%를 차지합니다. 이 랑게르한스섬에서 발생하는 종양은 랑게르한스섬종양, 또는 랑게르한스섬세포종양이라고 불립니다. 그러나 최근에 랑게르한스섬을 구성하는 내분비세포는 랑게르한스섬에뿐만 아니라 췌장의 외분비기능과 관계된 췌관상피와 선방세포 사이에도 존재한다는 것이 알려졌기 때문에, 넓은 의미에서 췌내분비종양이라고 불리는 일이 많아지게 되었습니다. 이들은 일반적으로 말하는 췌외분비기능을 담당하는 영역에서 발생하는 췌장암과는 구별됩니다.

췌내분비종양은 혈액 속에 호르몬을 과다하게 분비하는 기능성종양과 거의 호르몬을 생산하지 않는 무기능성종양으로 나뉩니다. 또 호르몬을 과다하게 분비하는 종양의 경우에는 그 호르몬 자체에 의한 증상이 나타나기 때문에 증후성종양이라고 부르며, 증상이 나타나지 않는 무기능성종양에 대해서는 무증후성종양이라고 부릅니다.

이와 관련된 드문 병으로서 다발성내분비선종양이라는 것이 있습니다. 이것은 가족성으로 유전되는 여러 군데의 내분비조직에 종양이 발생하는 질환으로서, 다른 내분비조직 (뇌하수체, 부갑상선 등)과 함께 췌장에서의 종양이 합병되기도 합니다.

2. 빈도

췌내분비종양은 췌장종양 전체의 약 2%전후를 차지하며 이 병에 걸리는 사람이 인구10만명당 1명 이하인 드문 질환입니다.

췌내분비종양의 대부분은 증후성종양으로서, 혈당을 조절하는 호르몬인 인슐린을 과다하게 생산하는 인슐리노마 (insulinoma)가 약70%, 가스트린을 과잉 생산하여 과산증에 의한 난치성소화궤양을 일으키는 가스트리노마 (gastrinoma)가 약20%를 차지하며, 기타 호르몬을 과잉 생산하는 종양이 나머지를 차지합니다.

또 나머지 종양의 발생 빈도는 훨씬 낮습니다. 무증후성종양은 췌내분비종양의 15%에서 20%를 차지한다고 알려져 있는데, 최근에 화상진단기술의 진보에 의해 우연히 발견되는 무증후성종양이 증가하고 있습니다.

[부작용에 대한 대책] 부작용의 종류와 정도는 치료법에 따라 다르며, 같은 치료법이라도 경우에 따라 달라집니다. 부작용의 대다수는 대증적인 치료를 통해 상당히 억제할 수 있습니다.

3. 증상

(1) 증후성종양

증상은 과잉 생산되는 호르몬의 종류에 따라 다릅니다. ■ 인슐리노마 (insulinoma): 유아에서 고령자에 걸쳐 넓게 분포하며, 남자보다 여자에게서 약간 많습니다. 저혈당증상이 일어나는데, 음식물을 섭취하면 회복됩니다. 가벼운 저혈당증세가 일어나는 경우에는 몽롱해지고 이상한 행동을 하게 되기도 합니다. 그래서 정신병과의 구별이 어려운 경우도 있습니다. 소아의 경우에는 경련과 혼수가 주된 증상이며 장기간동안 방치하게 되면 정신장해가 일어날 위험이 있기 때문에 빨리 진단하는 것이 중요합니다.

■ 가스트리노마 : 소아에서부터 고령자에 걸쳐 나타나며 남성에게서 약간 많습니다. 가스트린은 위산의 분비를 항진하여 복통이나 가슴이 쓰리는 등의 과산증상을 일으킵니다. 옛부터 졸림거-에리슨증후군이라고 불려왔는데, 좀처럼 낫지 않는 소화성궤양, 식도염이 있는 경우에는 이 질환에 대해서도 고려해보게 됩니다.

■ VIP생성종(혈관활성장폴리펩티드종양) : 소아에서부터 고령자에 걸쳐 나타나며 여성에게서 많다고 합니다. WDHA증후군이라고도 불립니다. 대량의 수양성 설사, 혈액 중 칼륨의 감소, 저위산 또는 무위산증을 나타내는 증후군입니다. 보통 하루에 3리터 이상의 설사를 하게 됩니다. 이 외에도 몇몇 증후성종양이 보고되어 있지만 드문 것들입니다.

(2) 무증후성종양

호르몬에 의한 증상은 없으며, 종양이 커지면 복부에서 종괴가 만져지거나 복통, 황달 등의 증상이 나타납니다. 30-50세 여성에게서 많이 발생한다고 알려져 있습니다. 최근에는 복부초음파검사를 통해 자각증상이 없는 무증후성종양이 발견되기도 합니다. 비교적 작은 종괴가 발견된 때에는 일반적인 췌장암과 감별하기가 어려운 경우도 있습니다. 4. 진단

증후성종양에서는 호르몬 특유의 증상에 대해 자세히 확인함으로써 이 질환에 걸렸을 가능성을 검토합니다. 또 원인이 되는 호르몬의 이상분비 여부는 혈액검사를 통해 확인합니다. 다음으로 종양이 췌장의 어디에 있는지, 몇 개가 있는지, 어느 정도의 크기인지, 확산범위는 어느 정도인지에 대해 자세히 조사하기 위해 복부초음파검사, CT검사, 혈관조영검사를 실시합니다. 증후성종양의 경우에는 다른 내분비조직에서의 종양을 합병하는 다발성내분비선종양이 아닌지를 알아봅니다. 무증후성종양의 경우 복부초음파검사, CT검사, 혈관조영검사, 내시경을 사용한 췌관조영을 실시해서 종양의 확산범위를 알아봄과 동시에 다른 췌장종양과의 감별진단을 해야 합니다.

5. 종양의 악성도

(1) 증후성종양

암은 일반적으로는 병소를 현미경으로 조사하는 병리검사를 통해 진단할 수 있습니다. 그러나 췌내분비종양은 병리조직학적으로 악성도를 판정하는 것이 매우 어렵습니다. 현시점에서는 간으로의 전이나 림프절전이가 있는 경우, 췌장 주변의 장기와 혈관에 종양이 퍼져 있는 경우를 악성이라고 판정합니다. 종양자체의 형태로는 크기가 큰 경우, 특히 5cm를 넘는 종양을 악성으로 간주하며, 종양 속에서 석회화가 일어나거나 중심부에서 괴사가 일어난 경우에도 악성이라고 보고 있습니다. 췌내분비종양은 췌장암에 비해서는 진행이 느리고 악성도가 낮다고 합니다.

증후성종양 중에서 인슐리노마 (insulinoma)는 80-90%가 양성인데, 나머지 다른 증후성종양은 반수 이상이 악성이라고 합니다. 또 무증후성종양은 양성과 악성의 비율이 거의 같다고 알려져 있습니다.

6. 병기

다양한 화상진단 및 외과수술을 실시한 경우에 이를 통해 얻은 소견을 참고로 하여 종양의 진전범위를 확인합니다. 이들 정보로부터 종양의 크기, 주위조직으로의 확산, 림프절전이, 간이나 다른 장기로의 전이 유무 등을 분석하고 그에 따라 종양의 진행도를 표시합니다. 췌장암과 같은 병기분류를 사용하고 있습니다.

7. 치료

췌내분비종양의 치료에는 외과적 치료와 내과적 치료가 있습니다.

(1) 외과적 치료

종양이 작은 경우와 인슐리노마 (insulinoma)에서는 림프절곽청을 할 필요가 없으며 기능을 보존하면서 종양을 절제하는 수술이 가능합니다. 증후성종양이나 무증후성종양 모두 전신상태가 양호하다면 외과수술을 실시합니다. 증후성종양에서는 호르몬 과잉분비에 의해 전신상태가 악화되기도 합니다. 그런 경우에는 내과적 치료를 통해 호르몬 분비를 억제하고 전신상태를 개선한 다음 수술을 합니다. 종양의 위치나 크기, 진전범위에 따라 수술방식은 달라집니다. 수술방식은 췌장암에 준하며, 종양이 췌장의 머리부분을 점거하는 경우에는 췌두십이지장절제를, 종양이 췌장의 몸통부분과 꼬리부분에 존재하는 경우에는 췌체부절제를 실시합니다. 췌두십이지장절제의 경우 종양의 확산정도에 따라서는 위장을 절제하지 않는 경우도 많습니다. 췌체부절제의 경우에는 비장을 함께 절제하는 것이 일반적입니다.

가끔 종양이 췌장의 가장자리로부터 돌출되어 있는 일이 있는데 이런 경우에는 종양과 함께 정상조직을 잘라내기도 합니다. 췌내분비종양은 진행이 느린 경우가 있으며, 이런 때는 완전히 종양을 절제하지 못하더라도 전이한 곳을 포함하여 되도록 많은 종양을 적출한 다음, 호르몬의 과잉분비에 의한 증상을 완화시키고 약물요법을 병용하는 치료를 하기도 합니다.

(2) 내과적 치료

내과적인 약물치료에는 과잉분비된 호르몬을 억제하고 증상을 완화시키는 항호르몬제치료와 종양에 대한 항암제치료가 있습니다. 항호르몬제는 증후성종양에서 일시적으로 수술 전에 전신상태를 개선하기 위해 사용되거나, 외과수술로 종양을 절제하지 못하는 경우에 장기간동안 호르몬의 과잉증상을 조절하기 위해 사용됩니다.

항암제는 외과적으로 종양을 절제하지 못하는 경우에 종양세포를 없애려는 목적으로 사용됩니다. 여러 항암제의 사용이 보고되어 있습니다만 표준적 치료라고 말할 수 있을 정도로 확립된 치료법은 없습니다. 증후성종양에서 항암제치료를 하는 경우에는 항호르몬제를 병용합니다.

8. 치료의 부작용

외과치료에는 수술 특유의 합병증이 있고 내과적 치료에도 약제 특유의 부작용이 있습니다. 진료를 맡은 의사는 최대한 그 발생을 회피하기 위해 노력합니다. 모든 치료에 있어서 치료를 시작하기 전에 의사로부터 충분한 설명을 들어 보셔야 합니다.

(1) 외과적 치료

췌두십이지장절제는 담관, 췌장, 위를 재건해야 하기 때문에 소화기외과 중에서는 큰 수술입니다. 특히 췌장의 재건은 다른 장기의 재건에 비해 봉합부전(봉합한 곳이 잘 아물지 않는 것)이 잘 일어나기 때문에 더 많은 주의를 요합니다. 췌체미부절제에서는 비장을 함께 절제하지만, 성인의 경우 비장을 적출하는 것 자체에서 오는 심한 부작용은 없습니다. 췌장을 수술하는 경우 정상적인 췌장의 조직도 종양과 함께 절제하는데, 잘라내는 정상조직의 크기에 따라서는 소화기능이나 혈당조절기능이 저하되기도 합니다.

(2) 내과적 치료

항호르몬제나 항암제는 경구적으로 투여하거나 정맥 내로 투여합니다. 약제에 따라 다양한 부작용이 나타나며 의사는 그 부작용에 대해 필요한 조치를 취합니다.

9. 예후

상복부의 중요한 부분이 절제 또는 재건되기 때문에 수술 후의 생활에 대한 지도가 중요하지만, 대부분의 환자의 경우 사회복귀가 가능합니다. 위를 보존한 경우에는 평상시와 같이 식사를 할 수 있습니다.

증후성종양에서는 외과적으로 종양이 완전히 절제되면 혈액 중의 호르몬이 정상으로 돌아오며 증상은 사라집니다. 췌내분비종양은 진행이 늦기 때문인지 보통의 췌장암에 비해 외과치료성과가 양호합니다.

그러나 남겨 둔 췌장이나 다른 장기에서 암이 재발하는 경우도 있기 때문에 수술 후에 정기적으로 점검하는 것이 매우 중요합니다. 또 절제가 불가능한 경우나 종양을 완전히 적출할 수 없는 경우에도 내과적 치료를 계속함으로써 오랫동안 생존할 수 있게 되기도 합니다.

Islet cell tumors of the pancreas are rare tumors and approximately 2500 cases of pancreatic islet cell tumors are seen in the United States each year. These tumors are often also called pancreatic neuroendocrine tumors.

Islet cell tumors of the pancreas are different from adenocarcinoma of the pancreas. These tumors are derived from neuroendocrine cells and tend to be slow growing tumors that are treatable even after they have metastasized. Islet cell tumors can produce dramatic symptoms since up to half of these tumors may secrete hormones that produce side effects due to excessive secretion of the hormones.

What are the different types of islet cell tumors?

The following types of islet cell tumors are recognized:

Non-functioning islet cell tumors. These are the majority of the islet cell tumors of the pancreas and may account for up to 60% of all neuroendocrine tumors of the pancreas. Patients with non-functioning tumors do not have any symptoms from excessive hormone secretion by the tumor since the tumor does not release any hormones into the blood. These tumors are often diagnosed because of non-specific abdominal symptoms that lead the physician to get a CT scan of the abdomen. Functional islet cell tumors. These tumors produce dramatic symptoms because of excess release of various different hormones by the tumor into the blood. The following types of functional islet cell tumors are recognized

Insulinoma: a tumor that produces excessive amounts of insulin. Gastrinoma: a tumor that produces excessive amounts of gastrin. Glucagonoma: an extremely rare tumor that produces excessive amounts of glucagon. VIPoma: an extremely rare islet cell tumor the produces excessive amounts of VIP. Somatostatinoma: an extremely rare tumor that produces excessive amounts of somatostatin. Non-functioning islet cell tumors

Non-functioning islet cell tumors are being identified more frequently because of widespread availability of CT scan imaging. These tumors have a histological appearance of islet cell tumors but do not produce any symptoms since they do not release excessive amounts of hormones in the blood.

Because these tumors are clinically silent and do not produce any hormones, they can grow silently for a long time before they are discovered. More than 50% of all non-functioning islet cell tumors are malignant (cancerous) and at the time of diagnosis the tumors are typically large and easily localized by CT scanning or MRI.

Work-up of non-functioning islet cell tumors at USC

At USC non-functioning endocrine tumors are worked up with the following studies:

High resolution thin section CT scan of the pancreas and the liver: This study is used for the diagnosis of the tumor, to assess whether there are any liver metastases and also to stage whether the tumor is confined to the pancreas or whether it has grown outside of the pancreas and is invading the surrounding blood vessels. This information is important for planning of surgery for this tumor. Octreotide scan: This scan is obtained prior to surgery to evaluate whether the tumor is positive on the scan. If the tumor is found to be positive on the scan then this is a useful study for following the patient after the surgery to detect early recurrences. Measurements of pancreatic polypeptide and chromogranin: Non functioning neuroendocrine tumors do not release any hormones that cause symptoms into the blood. Elevated amounts of two hormones that do not cause any symptoms: pancreatic polypeptide and chromogranin is found in the blood of more than 90% of patients. These two hormones are measured before surgery and during follow up after the surgery for detection of early recurrences. Treatment for non-functioning islet cell tumors

Surgical removal of non-functioning islet cell tumors is often curative. Our patients are therefore evaluated for surgery and all attempts are made to try and completely remove the tumor. The type of surgery depends on the location of the tumor. These tumors typically tend to be large and therefore enucleation of the tumor is usually not possible.

For tumors located in the head of the pancreas, a Whipple operation is usually indicated whereas for tumors in the tail of the pancreas a distal pancreatectomy is required. For very small tumors, organ preservation procedures such as a pancreatic head resection or a spleen preserving distal pancreatectomy may be possible options.

Our approach is to remove these tumors preferentially by laparoscopic surgery. An open procedure is usually offered to the patient if there is presence of metastases, for very large tumors (greater than 10 centimeters), if there is invasion of the major blood vessels around the pancreas by the tumor, and by patient preference. For all other patients, the laparoscopic procedure is offered as the treatment of choice.

We offer the following laparoscopic procedures for removal of non-functioning islet cell tumors:

Enucleation Distal Pancreatectomy Spleen Preserving Distal Pancreatectomy Central Pancreatectomy Whipple Operation Functioning islet cell tumors

Functional islet cell tumors often present in a dramatic fashion due to secretion of excessive amounts of hormones such as insulin, gastrin, or glucagon. These tumors tend to present at a very early stage when the tumor is tiny and is often not readily detectable.

Localization of these tumors is an important consideration since the tumors may be very small, only a few millimeters in size, when the patient presents with major symptoms related to over secretion of the hormones.

Pre-operative tests performed to detect the functioning islet cell tumors:

The following studies are offered at USC for detecting functioning islet cell tumors

Thin section high resolution CT scanning: The dramatic advances in CT technology now provide an opportunity to detect very small islet cell tumors. At USC the latest CT technology has allowed us to detect very small tumors that a few millimeters to as small as 5 millimeters in size. Endoscopic Ultrasonography: For small tumors that are not seen on CT scan endoscopic ultrasonography may identify many of these tumors. Octreotide Scanning: The octreotide scan is useful to localize the tumor and for following patients after the surgery whose tumors were positive on octreotide scanning for detecting early recurrences. Portal Venous Sampling: This test is available at USC and is used only in patients who have had prior surgery where the islet cell tumor was not detected. In this study a thin needle is introduced through the liver into the portal vein and blood is sampled from the branches the portal vein at various locations for the functioning hormone in the patient. Since all pancreatic blood drain into the portal vein, blood taken selectively from different branches of the vein allows the radiologist to pinpoint the area in the pancreas where the tumor is located. This study is very expensive and invasive and is therefore not indicated in patients who are undergoing surgery for the first time for their tumor. Intraoperative ultrasound: This is the most sensitive test for detecting small neuroendocrine tumors in the pancreas. The pancreas is directly imaged with a small ultrasound probe during surgery to detect tiny tumors in the pancreas. Experience of the surgeon: Experience of the surgeon is critical to localizing and detecting the islet cell tumors at the time of the surgery. The vast majority of the tumors are detected at the time of surgery, even where preoperative studies failed to localize the tumors prior to surgery. Treatment of functioning neuroendocrine tumors
